This includes submitting referrals, requesting home sleep testing services, securely transmitting records, receiving reports, and coordinating patient care and scheduling.The Portal is not intended for public or patient use unless expressly authorized.

2. Authorized Users Only

Access is limited to individuals authorized by their medical practice, healthcare organization, or Home Sleep Health Services, PLLC.

By using this Portal, you represent that you are authorized to act on behalf of your organization and will use the Portal only for legitimate healthcare purposes in compliance with applicable laws.

Unauthorized access or misuse is prohibited.

3. User Accounts and Security

Users are responsible for safeguarding login credentials. Users agree not to share usernames or passwords and to notify us immediately of any suspected unauthorized access.

Users should log out after use when appropriate and use secure devices and networks whenever possible.

We reserve the right to suspend access if security concerns arise.

4. Privacy and Protected Health Information

The Portal may be used to transmit patient information, including Protected Health Information (“PHI”).

Users agree to submit only information necessary for referral and treatment coordination and to comply with HIPAA and applicable privacy laws.

5. Accuracy of Information

Users are responsible for ensuring all submitted information is accurate, complete, and current, including patient demographics, insurance information, referral requests, uploaded records, and provider contact details.

We are not responsible for delays caused by incomplete or inaccurate submissions.

6. No Emergency Use

The Portal is not monitored continuously for urgent medical needs.

Do not use the Portal for emergencies or urgent medical situations. Call 911 or direct the patient to emergency care when appropriate.

7. Clinical Decisions

Use of the Portal does not guarantee acceptance of any referral, diagnosis, treatment plan, or timing of services.

All clinical decisions remain solely within the professional judgment of licensed healthcare providers.

8. System Availability

We strive to maintain secure and reliable access but do not guarantee uninterrupted availability. The Portal may be unavailable due to maintenance, outages, updates, or security events.

Users should maintain alternate communication methods when necessary.

9. Termination or Suspension

We reserve the right to suspend, limit, or terminate access for unauthorized use, misuse, inactivity, or security concerns.

10. Limitation of Liability

To the fullest extent permitted by law, Home Sleep Health Services, PLLC is not liable for indirect, incidental, or consequential damages arising from Portal use, outages, delays, or unauthorized third-party access beyond reasonable safeguards.

11. Changes to Terms

We may revise these Terms at any time. Updated versions will be posted with a revised Effective Date. Continued use constitutes acceptance of updated Terms.
